John Kanellopoulos, MD, presents a case of bullous keratopathy, vitreous in the anterior chamber, and glaucoma. First, he creates incisions with the LenSx Laser (Alcon Laboratories, Inc.). After explanting a silicone IOL and clearing vitreous from the chamber, he uses a reverse implantation technique in which an Artisan lens is inserted upside down and fixated in a retropupillary fashion. Dr. Kanellopoulos then performs a Descemet stripping automated endothelial keratoplasty and injects air bubbles to fixate the graft.
IRIS FIXATION OF A SUBLUXATED THREE-PIECE IOL UTILIZING A MODIFIED SIEPSER SLIDING KNOT

A 46-year-old man with a previous penetrating eye injury presented with a complaint of monocular diplopia and poor visual quality. He had undergone primary repair as well as lensectomy and IOL placement in the ciliary sulcus. A slit-lamp examination showed a subluxated three-piece IOL. This video demonstrates the use of a modified Siepser sliding knot to suture the existing IOL to the iris.
